New Patient Registration
We welcome new patients who live within our practice area. The reception team will advise new patients if their home is within our area. All new patients are asked to complete a registration form and health questionnaire prior to registration at the practice. These are available on the next page of the website called registering on line or by email from our receptionists. This can be emailed to you and then you can email the completed forms back top us or put them in the letterbox that is situated in the front wall of the building. The practice will not discriminate against any patient wishing to join our practice list on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
New patients are able to book a GP or Nurse appointment on the day following receipt of their registration forms. In certain circumstances an emergency appointment can be made.
Patients on repeat medications will need to speak to a doctor before getting their first prescription. An appointment will be offered as appropriate.
In normal circumstances new patients will usually be offered a New Patient Appointment upon registering with the practice but due to the COVID 19 pandemic these are not available at this time.
